    Massachusetts

Until now, PASS covered Massachusetts with an asset-driven monthly inspection that dates back to 2019. This month the state's own two logs arrived, each in two versions. One is a fillable smart form. The other is asset-driven. That means the inspection is built from the equipment PASS has on file for the site, so the tech checks each tank, line and sump that is actually there. The asset-driven versions follow the state minimum.

    • Massachusetts Monthly UST Inspection Log. The state's monthly log, new to PASS.

    • Massachusetts UST Sump Periodic Inspection Log. The state's periodic sump log, covering sumps that are continuously monitored and sumps that are not.

Both come with five service plans, one for each way a site can be on the schedule: monthly only, quarterly only, annual only, monthly with annual, and monthly with quarterly.

    New Hampshire

    • New Hampshire Visual Monthly Inspections for UST Systems. Both pages now answer with check marks and X marks, in place of the custom Yes/No choice the form used before.

    Virginia

Every Virginia testing form was brought up to the current PASS standard: check boxes, color coding, and automatic calculations where they apply.

    • Virginia Spill Bucket Integrity Testing (Hydrostatic/Vacuum Test)

    • Virginia Containment Sump/UDC Integrity Testing (Hydrostatic/Vacuum Test)

    • Virginia Containment Sump/UDC Integrity Testing (Low Liquid Level Test)

    • Virginia Overfill Prevention Equipment Functionality Test

    • Virginia Cathodic Protection System Evaluation Form

    • Virginia Annual Leak Detection Equipment Operability Check

    West Virginia

West Virginia got the same update, and the state also gained forms it did not have before. The four revised forms moved to the current PASS standard of check boxes, color coding, and automatic calculations where they apply. The interstitial monitoring test also gained a triennial service plan.

    • West Virginia Spill and Overfill Testing Report Form

    • West Virginia Interstitial Monitoring Testing Report Form

    • West Virginia ATG Testing Report Form

    • West Virginia UST Walkthrough Inspections Checklist

    New this month:

    • West Virginia UST Cathodic Protection: Galvanic. A galvanic cathodic protection evaluation for West Virginia, with a triennial service plan.

    • West Virginia UST Cathodic Protection: Impressed Current. The impressed current version, also with a triennial service plan.

    • West Virginia Annual Inspection Certification: AST Is Fit for Service. The certification for an aboveground tank that passes its annual inspection.

    • West Virginia Annual Inspection Certification: AST Is Not Fit for Service. The companion certification for a tank that does not pass.

A customer asked for the two aboveground tank certifications. They are open to every PASS service provider, with annual, triennial and ad-hoc service plans and a short guide to the state's application process.

    Montana and Mississippi

Both states now have their own Dri-Sump Containment Tightness Test Report. Each is a fillable smart form with a triennial service plan, open to every PASS service provider.

    • Montana Dri-Sump Containment Tightness Test Report. New to PASS.

    • Mississippi Dri-Sump Containment Tightness Test Report. New to PASS.

    Multi-State / Standards

The RP900 checklists follow the PEI recommended practice for inspecting UST systems. They offered a "v/" as an answer choice. This month all of them moved to a clean check mark and X.

    • PEI RP900 Monthly UST System Inspection

    • PEI RP900 Annual UST System Inspection Checklist

    • PEI RP900 Annual STP Inspection Checklist

    • PEI RP900 Annual Dispenser Inspection Checklist

    • PEI RP900 Annual Leak Detection Device Inspection Checklist

    • PEI RP900 Annual Fill Containment Sump Inspection Checklist

    • PEI RP900 Annual Transition Sump Inspection Checklist

    • PEI RP900 Annual Other Sump Inspection Checklist

Two more revisions came from service providers' requests. The Estabrook EZY CHEK Product Line Tester Data Sheet (Line Tightness Test) was updated, and the updated version is unbranded so any site can use it. The Mechanical/Electronic Leak Detector Certification was edited at a customer's request too.

    • UST Service Job Clearance and Site Safety Record. New this month. It holds a pre-service and a post-service checklist with sign-offs, so a visit has a recorded start and finish. A customer asked for it, and it is now open to every PASS service provider, with annual and triennial service plans.

    This month's headlines

October was a clean-up month. Virginia and West Virginia forms moved to the current PASS standard. The RP900 checklists dropped the "v/" answer for a plain check mark. Massachusetts finally has its own monthly and sump inspection logs. West Virginia also picked up two cathodic protection forms and a pair of aboveground tank certifications. Montana and Mississippi each got a Dri-Sump test report.

PASS now maintains a total of 1,522 service plans and forms within PASS Service, supporting customers across all 50 states and U.S. commonwealths.

We continue expanding and refining our inspection, compliance, and testing form library to support service providers and operators nationwide.

Running walkthroughs in Massachusetts, or testing in Virginia or West Virginia? Every form here is built into PASS service plans, so the right form shows up on the right site's schedule without anyone tracking it by hand. Talk to us about setting up your state's forms.
