Colorado Aboveground Storage Tank Monthly Visual Inspection Checklist. A new monthly visual walkthrough form for aboveground storage tank (AST) sites in Colorado, covering the routine visual checks an operator records each month.

Minnesota Notification and Status Forms. A set of four administrative forms that round out Minnesota’s underground storage tank (UST) library alongside the testing suite delivered last month:

New Hampshire Triennial UST Forms, Annual Cadence Option. Added an annual-frequency option alongside the existing triennial version on two New Hampshire forms, so schedulers can place each site on whichever inspection interval applies:

New Hampshire Spill Containment Integrity Test Form for UST Systems

Aboveground Storage Tank Interstitial Vacuum Decay Test. A new integrity test form that documents a vacuum-decay check of the interstitial space in double-walled aboveground storage tanks.

Vapor-to-Liquid Service Plan. A new service plan that supports a liquid-based testing approach in place of vapor-recovery testing, modeled to be comparable with an accepted regional form.

Iowa Monthly Walkthrough Update. A revision to the Iowa monthly walkthrough inspection form.

Monitoring System Certification (Annual): Inconclusive Result Option. Added an “Inconclusive” choice next to the Pass and Fail checkboxes for the Annual Monitor Certification in Section G, Line Leak Detectors, so a technician can record a result that is neither a clear pass nor a clear fail.

Oklahoma Testing Forms. Updates to Oklahoma’s release-detection, containment, cathodic protection, and tightness testing forms, a suite of nine:

Texas Impressed Current Rectifier Inspection Log Sheet. A new 60-day rectifier inspection log for impressed-current cathodic protection systems in Texas.

PEI RP1200 Appendix C-2 Triennial Form. A new triennial inspection form built to the PEI/RP1200 Appendix C-2 procedure.

Meter Calibration Form. A new meter calibration form matching a customer-required calibration format for fuel dispensers.

Kansas UST Forms. Three additional Kansas state forms:

Static Torque of Rotatable Phase I Adaptors (TP-201.1B), Triennial Version. Created a triennial version of the TP-201.1B static torque test form for rotatable Phase I vapor-recovery adaptors, adding to the existing annual version.

Among this month’s additions, North Dakota received a complete new suite of twenty-four inspection, testing, and recordkeeping forms, the largest single-state buildout of the month, and Oklahoma’s nine testing forms were refreshed across release detection, containment, cathodic protection, and tightness testing. We also expanded Minnesota coverage with four new notification and status forms, added an annual-cadence option to two New Hampshire triennial forms so sites can be scheduled on whichever interval applies, and introduced new tests including an aboveground storage tank interstitial vacuum decay check and a set of fillable pressurized and suction line-test forms.

PASS now maintains a total of 1,459 service plans and forms within PASS Service, supporting customers across all 50 states and U.S. commonwealths.
